Once your request has been approved by our executive team, I agree to the following:
-Tables and chairs will be put back in accordance to the picture diagram in each room.
-Tables and counters will be wiped down.
-Floors will be swept or vacuumed (brooms and vacuums are found in the janitor's closet).
-Trash will be taken to dumpster (this includes all bathrooms and kitchen).

I also understand that there may be custodial and/or technical fees which may be required for this event, and these will be communicated to me before this event is approved.

I agree that Fellowship Chapel health and safety guidelines will be enforced for the duration of this event.

Finally, I understand that events submitted for approval without adequate lead time are more subject to rejection (one month lead time minimum is preferred).
